Millicent's questline in Elden Ring is one of the most compelling NPC narratives in the Lands Between, offering rich lore tied closely to Malenia and Scarlet Rot. Tracking her down requires traveling across multiple regions, from the scarlet swamps of Caelid to the heights of Elphael. Keeping up with daily gaming news helps players stay informed on the best routes and strategy guides for complex FromSoftware quests.

Starting the Journey in Caelid

Your encounter with Millicent begins in Caelid at the Church of the Plague, located atop the cliffs overlooking Sellia. Before she will talk to you meaningfully, you must assist Gowry in his shack to obtain the Unalloyed Gold Needle. After retrieving and fixing the needle, administer it to Millicent to alleviate her rotting affliction and advance her narrative.

Moving to Altus Plateau and Mount Gelmir

Once healed, Millicent moves to Gowry's Shack briefly before venturing north to the Altus Plateau. You can locate her standing near the Erdtree-Gazing Hill Site of Grace, where she expresses a desire to fight despite losing her arm. To aid her combat abilities, travel to the Shaded Castle in northern Altus to retrieve the Prosthesis-Wearer's Heirloom item from a chest.

The Windmill Village Encounter

After giving Millicent her prosthetic arm, she travels further north to Dominula, Windmill Village. You must defeat the Godskin Apostle boss residing at the top of the village to clear the path. Rest at the nearby Site of Grace, and Millicent will appear to thank you for your assistance while pledging her blade to your cause.

Mountaintops of the Giants and Elphael

Millicent next moves to the Mountaintops of the Giants, resting at the Ancient Snow Valley Ruins Site of Grace. Speaking with her here sets up the final leg of her story in the hidden area of Miquella's Haligtree. She eventually moves to the Prayer Room inside Elphael, Brace of the Haligtree, where she reflects on her destiny and connection to Malenia.

The Final Choice at the Haligtree Drainage Channel

The quest reaches its climax near the Drainage Channel Site of Grace in Elphael after defeating a Defiled Root Monster. Player summon signs will appear on the nearby shore, allowing you to either aid Millicent against her sisters or invade her. Choosing to assist her unlocks key talismans and allows you to retrieve the Unalloyed Gold Needle for alterative ending choices.
